BKGB Sports Camp Waiver & Emergency Contact Form

Before your camper can participate in BKGB Sports Camp, a parent or legal guardian must complete the required Waiver & Emergency Contact Form.

Important: This waiver does not register your camper for camp. Camp registration and the $10 admission must be completed separately through the Camp Registration page.

Please complete one waiver for each camper attending.

Reminder: Your camper is not cleared to participate until both camp registration and the waiver are complete. Campers will not be allowed to participate without a completed waiver.
